In 1990, he began practicing medicine at Tshilidzini Hospital and later, in 1995, became a professor and head of the Department of Otorhinolaryngology at the University of Pretoria.

In the year 2000, Dr. Tshifularo achieved a significant milestone by becoming the youngest and only Black professor of Otorhinolaryngology in South Africa.
